I am having the same exact issue with Cantabile 3 using OPX v1.3.1. @Tweaker @brad was this issue fixed by using a later version of Cantabile?

Yes, OPX v1.3.0 and 1.3.1 work fine in the latest builds. Earlier builds had problems running some vst3 plugins.

There have been fixes made in this area. To test this for sure for yourself, you can grab the latest Cantabile Lite and give it a go.

OK. Updating my Cantabile to the latest version (v4) seems to have solved this issue for me Thank you.
