I’ve noticed that if I load an MP3 into a media player, set it to Master, Cantabile will beat sync - the metronome blinks at the same time as the song.
Is this a coincidence? If it isn’t (and I hope it isn’t) - can the first beat be adjusted? Sometimes it doesn’t find the first beat - probably due to a weird intro on the mp3, and throws other arps running in the song…

Each audio file has a setting to specify tempo and time signature - double click media player, double click the file and you’ll see the settings. It defaults to 120bpm & 4/4, unless the file name has something that looks like a tempo indicator in it. eg: “Funky Beat 130bpm.mp3” it’ll pick up the 130bpm.
ie: it’s not really syncing - it just has an assumed tempo.
As for the adjusting the starting point - that’s not currently supported (but I do have plans for a for a couple of different improvements in this area). In the meantime, unfortunately you’ll need to edit the audio file so it’s properly aligned at the start of the file.
